If you’re using a third-party app to open the zip file, they sometimes have trouble dealing with Dashboard widgets correctly. Try downloading it again, then right-click (or control-click) on the zip file, and choose Open With > Archive Utility. Then double-click the Delivery Status.wdgt file to install it.
